Navigating Your IRA Application: Tips and Instructions
Below, you'll find guidance for completing your IRA opening paperwork.
Documents may be submitted via email, fax, or US Mail

Fill in your personal information in Part 1.
Indicate the source of funds in Part 3 (IRA Transfer, Rollover, or Personal Contribution) without specifying the contribution date.
Complete Part 4 with your personal details and attach a CLEAR copy of your photo ID.
Specify Primary and Contingent Beneficiaries in Part 5.
If not naming your spouse as Primary Beneficiary, include their signature and a witness's signature in Part 6.
Sign as the IRA Owner in Part 7 and have the form notarized.
Return pages 1 and 2 with your photo ID copy. (Please note that you are not required to return pages 3-15 of the IRA Application, keep these for your records).

Send check for $175 payable to "Premier Trust, Inc."

Include a clear copy used during notarization.

Complete Part 1 with personal information and account type.
Leave the “Account Number" blank; we'll assign it later.
Indicate your relationship to the IRA in Part 3.
Provide personal information and the account number of the transferring IRA in Part 4.
Specify where to send Transfer Request paperwork in Part 5.
Complete Part 6 if you are over 72.
Enter the transfer amount in Part 7, ensuring it covers fees and minimum balance.
Sign as the "Recipient" in Part 8 (no notarization needed if transfer amount is under $1,000,000.00).

Sign as the IRA Owner in Part 4.
